Care Partner or CBRS Case Manager– EverCare Mobile Health

Location: Eastern Idaho - Idaho Falls
Schedule: Part Time to Full Time 
Compensation: $24 - $26 per hour

  • $18- $28 per billable hour

Key Responsibilities
  • Assist with Community Health Integration and navigation for long term care residents. 
  • Conduct assessments, treatment plan reviews, and progress notes.
  • Maintain accurate and timely documentation in compliance with payer and company standards.
  • Participate in supervision and team meetings.
  • Collaborate with other treatment providers to ensure comprehensive care.
  • Adhere to HIPAA privacy and compliance regulations.
  • Assess emergency situations and escalate concerns as necessary.
  • Provide consistent support to clients, promoting personal growth and accountability.


Qualifications

  • Bachelor's degree in Psychology, Social Work, Healthcare preferred, or a helping field required
  • CPR certification (preferred).
  • Ability to pass an enhanced IDHW/ORHW background check.
  • Valid driver’s license, reliable transportation, and proof of insurance.
  • Experience using electronic health records (preferred).
  • Comfortable working in a community-based setting with experience in long term care and would prefer Peer Support Certification or Medical Assistant Certification.
Compensation: $20.00 - $28.00 per hour
